Start with the federal requirement, then build for the operation
Federal motor-carrier rules require commercial motor vehicles to be systematically inspected, repaired, and maintained so that parts and accessories are in safe and proper operating condition. Periodic inspection requirements create another layer of control. A strong fleet does not treat the annual inspection as the maintenance program; it uses routine PM, driver inspections, shop inspections, defect reports, roadside findings, and repair history throughout the year.
PM intervals should reflect equipment type, mileage, engine hours, duty cycle, geography, load, idle time, environment, manufacturer recommendations, and fleet experience. A refuse truck, linehaul tractor, dump truck, city delivery unit, and low-mileage seasonal vehicle may need different triggers even when they share a chassis brand.
Define what every PM inspection must accomplish
A PM service should do more than change fluids and filters. The inspection portion needs a defined scope covering safety-critical systems such as brakes, steering, suspension, tires, wheels, hubs, lights, coupling, frame, exhaust, fuel system, electrical routing, emergency equipment, and other items appropriate to the vehicle. Technicians should know the acceptance criteria and where to find specifications.
Use checklists as memory aids, not substitutes for thinking. A technician who notices an unusual wear pattern, fresh rub mark, loose harness, repeated leak, or component out of position should have room to investigate even if the checkbox is technically passable. Good PM catches developing conditions that do not yet produce a driver complaint.
Create defect severity levels
All defects are not equal. Use repeat failures to improve the program
A road call is not just an invoice. Code failures consistently by component, unit, mileage or hours, position, vendor, and cause when known. Review repeat patterns monthly. Three alternators, multiple wheel seals, repeated air-line rubs, recurring lighting failures, or repeated brake defects on similar units may indicate specification, installation, routing, vendor, training, or interval problems.
Trend data can justify changing PM intervals, stocking different parts, issuing a service bulletin, retraining technicians, or modifying equipment. Without that feedback loop, a fleet can repair the same underlying problem hundreds of times while calling every event unexpected.
Qualifications, tools, and records protect the program
Safety-critical work requires competent personnel and appropriate tools. Brake inspectors have specific federal qualification requirements, and other work may require manufacturer training, electrical diagnostics, torque equipment, lifting procedures, or specialized knowledge. Maintain training and qualification records and make current service information available to technicians.
Maintenance records should be complete enough to show what was inspected, what was found, what was repaired, when it was returned to service, and who performed or approved the work. Records support compliance, but their larger value is operational: they let the fleet see component life, repeat failures, cost, and risk over time.
A supervisor’s field audit: what to look for
A safety rule becomes reliable only when supervisors can observe it in normal work. Conduct short field audits in yards, at terminals, during ride-alongs, or through appropriate video review. Look for the behavior the policy is supposed to produce, then ask the employee to explain the reason behind it. A person who understands the hazard is more likely to make the correct decision when the exact situation is not written in a checklist.
Record both strengths and gaps. If several employees make the same mistake, examine training, equipment, work design, or expectations before treating it as a collection of individual failures. Repeated behavior usually has a repeated cause. Correcting the system is more durable than issuing the same reminder every month.
Training questions that reveal real understanding
Instead of asking only whether an employee completed training, use scenario questions: What would make you stop the job? What change would make this condition unsafe? Who do you call when you are unsure? What evidence would you put in a defect or incident report? What is the safest alternative if the planned method cannot be used? These questions expose whether the employee can apply the rule under pressure.
Refresher training should use actual fleet events, photographs, inspection findings, near misses, and equipment examples. Remove names when appropriate and focus on the decision points. People learn faster from a familiar tractor, trailer, route, dock, or component than from a generic slide that does not resemble their work.
Documentation should support safety, not bury it
Keep records that help the next person make a decision. Use consistent defect codes, clear descriptions, photographs when useful, unit and component location, dates, repair disposition, and responsible personnel. Avoid turning documentation into so many fields that employees rush through it. The best record is complete enough to reconstruct the condition and simple enough that people will actually use it.
Finally, review the data. A folder full of completed forms is not a safety program. Trend defects, near misses, roadside events, coaching observations, and repeat repairs. Share the lessons with drivers and technicians so they can see that reporting produces action. That feedback is what keeps a safety system alive.

Implementation works best when the rule appears in the same places where the work is planned. Put the expectation in orientation, driver and technician handbooks, dispatch procedures, inspection forms, supervisor coaching, and incident review. Safety leadership therefore has to align scheduling, maintenance availability, staffing, and performance measures with the behavior it asks employees to use.
Set a simple escalation path. Employees should know who can answer a technical question, who can authorize a vehicle hold, how to request roadside help, and what to do after hours. Uncertainty becomes dangerous when the employee believes the only available option is to improvise. A clear phone number, duty manager, maintenance contact, or documented decision tree gives the employee a safer alternative.
Close each event with learning. If a driver stops for a concern and the vehicle is later found safe, do not treat the stop as wasted time. Review what the driver observed and explain why the condition was acceptable. If the concern was valid, show how it was corrected. Both outcomes improve judgment. The objective is a workforce that reports early, asks useful questions, and recognizes developing hazards before they reach the point of failure.
